You have the ability to edit the existing form. Choose your report, and note the suffix of the form (eg. xxx.CombinedStatement). Then instead of running the report go to Edit > Form Design. When you’ve made all the changes you want, be sure to save the document with the same suffix that you noted earlier.
That is part of your report form and you can remove it yourself
