A new employee was entered but the new W4 page was not completed. The employee then had no Federal Income Tax taken out.

Default W4 to highest withdrawal, or have a warning that no W4 information has been entered.

Not sure what version you are on but Sage does warn if you try to save without entering information on the w-4 screen.
Also, in 5-2-1 you need to enter the W4 info under the W4 tab only and don't do anything in the grid under the "calculations tab" under the "Federal Income" section from your Payroll Calculations section. Hope this makes sense.
