AZ misc tax on the A4 form start at box 1=.8%, box 2=1.3%, box 3=1.8%, box 4=2.7%, box 5=3.6%, box 6=4.2%, box 7=5.1%. In Sage emp setup, Taxes the misc tax codes begin with 1 = 1.3%, 2=2.7%, 3=3.6%, 4=4.2%, 5=5.1%, 6=1.8%, 7=.8%.
